Controlling chronic disease progression presents the greatest opportunity to reduce hospitalizations, improve patient health and manage healthcare costs.
Historically, healthcare providers attempted to manage soaring healthcare costs by focusing on controlling individual components of care. These efforts were largely ineffectual because, reducing the costs of some components often led to increases in others.

Our hands-on approach to chronic disease management is, perhaps, the hallmark of QMed's model. We believe that physicians are the key to quality health care. Our nurses and Care Managers are in direct, personal contact with physicians and patients from the initiation of the engagement process. They become an integral part of the multi-disciplinary team for every patient case. Our Care Managers interact with patients on a regular basis, teaching them how to self-manage their disease and monitor their personal progress.

Our robust database of information contains data from a large array of sources, creating an electronic medical record or fingerprint of each patient. Through our sophisticated technology platform, this information is analyzed and compared to national guidelines of evidence-based medicine which are provided to physicians as up-to-the-minute recommendations.
