The core of our Health e Living program is an individual assessment and personalized care plan, with participants fully engaged through convenient interactive voice response and online web tools. Our remote resources are integrated with our Personal Health Coaches to provide needed hands-on guidance, motivation and monitoring every step of the way:

  • Participant Profile: Customized to participant’s personal background, information, health concerns and goals
  • Health Assessments: Used to gauge and monitor risk and prevention of health conditions associated and impacted by weight
  • Nutrition Guide: Built to provide diet and diet structure, caloric range, food preferences, information on how to eat outside the home, and a recipe center for new cooking ideas
  • Exercise Guide: Established personalized plan for aerobic and weight training
  • Special Needs: Center to address concerns as it relates to other aspects of your health and wellness
  • Learning Center: Online support for the empowerment and path to living a happy, healthy and productive life with free information and courses on a variety of health topics
  • Progress and Results: Input of information for tracking and reporting individualized program progress; the system network submits reports and allows selection of healthcare providers that need to be informed of a participant’s health
