Health e Weight

Health e Weight is a highly personalized and supportive program, designed to address the physical, behavioral and psychological needs of those suffering from Obesity—along with the rising incidence of medical conditions related to obesity. These conditions include type 2 diabetes, coronary artery disease, depression, hypertension, sleep apnea, stroke, arthritis and cancer of the reproductive organs. Additionally, our program addresses extreme circumstances that can result from obesity, including the decision to have invasive procedures such as gastric bypass surgery and the related pre- and post-surgery challenges that patients face.

We actively engage participants in their care management plan, including personal health coaching, telephone interventions and interactive web-based support to:

  • Reinforce lifestyle changes as well as receive guidance from your Personal Health Coach
  • Ensure ongoing motivation and educational support to achieve goals
  • Review diet and calorie logs with your Dietitian
  • Monitor weight plan and exercise goals with your Dietitian
  • Report and receive information using your personalized web site and interactive phone
  • Track and evaluate weight, glucose and blood pressure-as well as other key measures

In the event that you are already receiving clinical treatment, we coordinate with your physician and provide direct access to your electronic health file and information through a password protected, HIPAA compliant web site. This makes it efficient and effective for your physician to participate in your treatment plan and monitor your results on your road to improved health.
